Not only are regular eye exams important for maintaining your vision, they can also help identify serious health conditions. At Visionary, we want you to look stunning in your new glasses, but we also want to do our part to keep your eyes healthy for years to come.
We’d like to give you some information to help make your visit with us as smooth as possible.

New Patients
If you’ve already set up your appointment, make sure you’ve set up your patient portal here.
Make sure we have all of your relevant information for your vision insurance. For a full list of what is needed, including what insurances we take, go to the “Insurance” section of the FAQ page.
Please bring any eyewear you currently use: glasses, reading glasses, contact lenses, and prescription sunglasses.
Please bring a physical copy of your medical insurance, so that any future visits requiring medical attention (infections, injuries, etc) will be billed to the proper insurance company, and we have all of your copay information up to date.
Returning Patients
If you’ve already set up your appointment, make sure your information is up to date in our patient portal.

All Patients
HIPAA
At Visionary we respect your right to privacy, and we are committed to protecting your health information. We also support your right to access your medical records, and fully comply with the Health Insurance Portability and Accountability Act (HIPAA). Our Patient Portal meets the HIPAA Security standard, safeguarding your electronic health records.
Authorization Release
If you wish to have your health information forwarded, please print and fill out the Authorization Release form, then submit it to the appropriate provider.
Cancellation Policy
A $25 cancellation fee will be charged to patients who cancel appointments with less than 24 hours notice, and a $40 no show fee will be charged to patients who do not show up to their appointment.
