As our first group project, we’re going to work together to keep us all safe. Our patients, our employees, our doctors, and our neighbors all deserve to be safe and healthy, so we are going to adopt the guidelines of the City of Chicago Vaccine Requirement.
We are already currently appointment-only, but we are now requiring proof of vaccination in order to attend your appointment. You can see below for a list of acceptable forms of verification, and you can go to this link if you’d like a digital copy for your phone from the IL Department of Public Health.
Please note: We reserve the right to refuse non-emergency patients if they are not vaccinated.We will see patients with medical eye emergencies, regardless of vaccination status.
